To schedule a State Safety Inspection for licensing, complete the State Safety Inspection Request Form below.  Safety Inspections are required to obtain or renew state licensing.  A fee of $144.00 must be paid prior to scheduling the inspection.

Instructions will be sent to the requestor once we’ve processed the application to make payment and inspection scheduling.
