To schedule a State Safety Inspection for licensing, complete the State Safety Inspection Request Form below.  Safety Inspections are required to obtain or renew state licensing.  A fee of $120 must be paid prior to scheduling the inspection.

Instructions will be sent to the requestor once we’ve processed the application to make payment and inspection scheduling. Please allow up to 24 hours from the time of submittal for processing.
